The Hospital Social Work Service provides a full assessment and care management service to all Newham residents admitted for acute hospital care under the requirements of the Community Care (Delayed Discharge etc) Act 2003. The Service is based at Newham University Hospital Trust in Plaistow but also provides a service to the Cazaubon Unit, a multidisciplinary community rehabilitation unit based in East Ham

Demand is high and the work is complex and challenging. The emphasis is on robust risk management to support people to live in their own homes. The Team is committed to multi-disciplinary working and user/carer involvement in the discharge planning process.

Annual appraisal and regular supervision is provided as well as training opportunities. The department is leading the way in social care by introducing a range of technology such as tablet PC’s and mobile working enabling social workers to record assessments electronically during visits with service users. The new technology reduces the amount of administrative duties and increases the time social workers can spend servicing the needs of service users.

You should be qualified social worker, with experience of social work/care management with adults and possess a sound knowledge of current professional and practice issues relating to adults and older people.

You need to have registered your social work qualification with the GSCC.
