Fixed term until 31 March 2014.
To provide Occupational Therapy Support which enables disabled people to maintain or achieve optimum functional ability and independence. To provide advice, support and training to staff and individuals relating to their needs to ensure positive and appropriate outcomes for disabled people.
You will promote and safeguard the welfare of vulnerable adults.
The role will undertake specialist OT assessments to determine eligibility, mental capacity, physical capabilities and ongoing needs including for e.g. independent medical assessments (Blue Badge) and re-housing.
You will arrange for the provision of services and equipment in accordance with Council policies and procedures and agreed protocols when working within a multi-disciplinary setting.
You will be involved in individuals, their families and carers in the development of assessment outcomes to determine options available for the customer to determine their desired outcomes.
You will document casework accurately to enable accurate assessments to be made.
